Texas hospitals incurred more than $1 billion in health care costs for patients not lawfully present in the United States during fiscal year 2025, according to new data obtained from the Texas Health and Human Services Commission.
The figures were collected under an executive order issued by Gov. Greg Abbott in August 2024, which requires hospitals to report the cost of inpatient and emergency care provided to individuals in the country illegally. Under Abbott’s order, hospitals are also required to inform patients that responses regarding immigration status will not affect their care, as required by federal law.
Statewide totals show 313,742 hospital visits from patients not legally present in the U.S., costing hospitals $1.05 billion during the reporting period. The largest share of the expense—more than $565 million—came from inpatient discharges for non-Medicaid and non-CHIP patients.
Emergency department visits accounted for roughly $230 million, while total inpatient care exceeded $820 million, underscoring that long-term hospitalizations, not emergency treatment alone, are driving much of the cost.
Although hospitals are required under federal law to deliver the care, unpaid medical costs are ultimately passed along to Texans. Taxpayers absorb the burden through higher insurance rates, public hospital funding, and state health programs.
Those undocumented are paying taxes, so it ain’t all on the rest of us. What it is is that people in power, the MAGAs, like to gift billions to the people who fill their campaign coffers.
Breakdown of Tax Contributions in Texas
According to a 2024 report by the Institute on Taxation and Economic Policy (ITEP), approximately 1.9 million undocumented immigrants in Texas paid taxes across various categories in 2022:
- Sales and Excise Taxes: Undocumented Texans paid approximately $2.8 billion in sales and excise taxes, the largest share of their state and local tax contributions.
- Property Taxes: Around $1.8 billion was paid through property taxes (both by homeowners and through rent, as property taxes are often passed on to renters), which helps fund public education and local government services.
- Personal and Business Taxes: Undocumented immigrants paid an additional $0.2 billion in various personal and business income taxes.
Federal Tax Contributions
Undocumented immigrants are also required to pay federal taxes and do so in large numbers, often using an Individual Taxpayer Identification Number (ITIN) issued by the IRS. In 2022, undocumented immigrants nationwide paid $96.7 billion in federal, state, and local taxes.
A significant portion of federal tax dollars goes towards programs they cannot access:
- Payroll Taxes: Undocumented immigrants paid approximately $25.7 billion into Social Security and $6.4 billion into Medicare in 2022, programs from which they are currently barred from receiving benefits.
